Not defending O-care at all, but I've worked with medical billing departments in a professional capacity(and on the receiving end too) and for years many of them have been what Clint labeled a Cluster Mess. Don't know how in the world patients are supposed to keep up with how to get it right when the "professionals" often seem clueless.
My mother got bills for my father 3-4 years after he passed, it took getting an attorney involved to get that straightened out. Not talkin' care he'd received but stuff supposedly administered after he was in the ground. It's a shell game with all the different insurance companies and medicare particulars.
I remember an HR rep telling me that if I went in for a check-up to make sure they listed it as a "wellness exam" because BCBS wouldn't pay for a "check-up" but a "wellness exam" was covered. That's what happens when the piranhas are grubbin for every tiny piece of flesh they can get their teeth on.
BTW, Cluster Mess is a noun in this case
