I had my final diagnosis and today. The MRI shows a herniated and degenerating disc (L 4/5) is the cause of all this. I have "episodes" where it breaks down enough to cause a chain reaction in the nervous system that makes my lower back muscles go into extreme hyper-cramping. They pull themselves apart until I get treated (typically in the ER). That's the event that puts me on my back for about 3 weeks. There's no operation to fix it, so the answer for relief or a pseudo cure, will be cortisone shots. Until then I'm on a battery of drugs that's gets me on feet so I can at least putter around the house. After the shots I should be relatively normal soon.
