That's Talon 2.0 by *********'s I think it's on his web site. I had a small hand in the idea of development of the grub size loved the tail of his first creation can't remember what he called it,I just wanted a bigger grub to handle a #2 hook he suggested a sink-o type body and bingo the birth of the Talon 2.0 I clam the name I made it up. Finally it catches fish period.
