Had to remove one from my dad's arm. His was a self inflicted wound while showing off a little! I had to push the hook through, cut the point and barb off and then back it out. His was on the backside of his forearm near the elbow. He could barely see it.

Hooks in my hands, I just use a set of needle nose and a quick yank.
