Anyone that wants an entry form, can get one by sending me an email at [email protected] or call me at 731-336-3550. If you are in the Paris area, all the bait stores or just about any business by the lake has them. You can just pick one up. If any of the stores are out of them, let me know and I will get more to them. Thanks
