You are in the right place, but I don’t know of a mold like that. They may exist but I don’t know of where. Have you tried Zeiners?
Seiners seems to only carry do-it molds and they don’t have what I’m looking for.
Fat guys fishing has a mold that seems to be the closest to what I want but uses a 90 degree hook with a ball head. I’m really wanting the in-line hook eye and flat head design like Charlie Brewer uses. Wish in one hand and in the other I’m starting to think.

You won't find a mold like that as it's owned by Charlie Brewer. The only way is to make one out of plaster of paris or RTV.
If one is made from plaster of paris, how many pours can you expect to get?
Unsure about plaster but on YouTube are many videos showing folks using silicone to form molds, and pouring into those to make lure. There are different types of silicones for this use and it would be well worth the effort to read some and ensure you get the best. I imagine you could make a handful of molds rather easily and get enough pours out of them to get by just fine. If you do this try to pour at the lowest temp you can. 650 F would be what I would want to try to use. Thermometers to measure are not very much money.
I wonder if it will be worth it. I mean doing this project vs just buying the jigs. Bet it depends on how many you are losing.
Best option for the time being is probably just buying some.
I could not find Tru turn style hooks a few years ago. No where had any. A bend in a hook shank is all it is. A pair of needle nose pliers when make that bend quickly.
