I have never seen any for sale anywhere

It's not a C-55 but pretty close:
Fishing Lures Crankbaits C628 4 inch 2/3 oz – wLure

Wlure has them occasionally. I have probably a dozen or so.

Mine are blanks though, with black on back

WLure has some painted versions.
