Check inside of hub where it mounts for wear. Also i lost some of those off brand caps until i went back to real bearing buddies. Had one set that would actually push out of hub when greasing. you also might try cleaning hub and cap with brakekleen real good, use starting fluid to clean off breakleen, and then put on a coat of green loctite bearing mount or red loctite.
When you sand blasted the hubs you may have made just enough difference where the cap isn't seating evenly and vibrates out. My 2 cents.

If they are the spring loaded kind, u have to stop putting grease in them when the spring moves no more than 1/4 inch or they have a 90+% chance of coming off. I've had this happen before and found that to be the case using the same hubs.
I think it is, stop just when the spring starts to move. Anyway, I feel sure thats what caused my problems..
